Abstract

The operations of cargo loading and unloading on chemical tankers have a high level of risk because they involve hazardous liquid cargo and complex technical systems. These conditions can cause operational disruptions if they are not supported by proper supervision, coordination, and optimal operation of the cargo system, one of which is the occurrence of overflow and overpressurized tank. This study aims to analyze the causes of overflow and overpressurized tank and their impacts on cargo loading and unloading activities on the chemical tanker MT. Marina Aman. This study used a qualitative approach with a descriptive case study method. Data were obtained through observation, interviews, and documentation studies. The results showed that the overflow occurred due to an abnormality in the Pump Remote Control Assembly of the 4S cargo tank pump, which caused the pump to operate without a command and resulted in cargo being transferred to the 2S tank without being noticed. This condition was worsened by the malfunction of the remote tank gauging system on the 2S tank and the failure to report an early indication in the form of an abnormal pump sound. Meanwhile, the overpressurized tank was caused by the less optimal performance of the Pressure/Vacuum (P/V) valve and pressure sensors, as well as delays in maintenance. These results show that the occurrence of overflow and overpressurized tank was not caused by a single factor, but was influenced by a combination of equipment conditions, monitoring systems, maintenance, and human factors. The impacts included disruption of cargo loading and unloading activities, damage to the tank structure, and operational losses for the company. Therefore, improvements in equipment maintenance, cargo system monitoring, and more effective communication and reporting are needed during cargo loading and unloading activities.
